Discharge Planner – Little Rock Midtown Clinic

General Description of Duties and Responsibilities:

In this critical role, the Discharge Planner will work closely with our clinical provider team in the Little Rock Midtown clinic, focusing on reviewing care plans, addressing social determinants of health, and connecting patients with the necessary resources. This position is vital in linking clinical services with community support to contribute to the holistic well-being of our patients.

Specific Duties and Responsibilities:

  • Patient Relations:
    • Participate in post-provider visit consultations to understand and plan for patient care needs.
    • Serve as the primary system navigator and point of contact for patients and families.
    • Update primary care charts with information on specialist consults, hospitalizations, ER visits, and engagements with community organizations.
    • Coordinate with specialty services, 340B pharmacy program, behavioral health services, and other essential services.
    • Liaise between patients, primary care providers, insurance companies, and other healthcare entities.
    • Facilitate patient access to healthcare benefits, including transportation and medication assistance.
    • Uphold patient confidentiality and aim for the highest level of patient satisfaction.
  • Qualifications:
    • High school diploma or equivalent required.
    • A minimum of two years of experience in a medical office environment.
    • Bilingual abilities (Spanish) are preferred but not required.
    • Proficient with Microsoft Office suite and familiar with network systems.
    • Familiarity with medical terminology and healthcare communication protocols.
    • Exceptional organizational, written, and interpersonal communication skills.
    • Ability to travel between locations and attend external meetings as necessary.
