About the Role
Healthy Connections is seeking a Community Health Worker (CHW) to help patients navigate healthcare services and community resources. The CHW will provide social support, patient education, and advocacy to improve health outcomes. This role is ideal for someone passionate about serving the community and reducing barriers to care.
Key Responsibilities
- Assist patients in accessing healthcare services, insurance, and social resources.
- Conduct outreach activities and provide health education.
- Collaborate with local organizations to address social determinants of health.
- Maintain accurate documentation of patient interactions.
Qualifications
- Knowledge of community resources and social services.
- Strong communication and interpersonal skills.
- Experience in outreach or community engagement preferred.
- Bi-lingual (Spanish) preferred.
- Valid driver’s license and reliable transportation.
