To support this mission, we offer a Sliding Fee Discount Program for eligible patients, ensuring that no one is denied care due to an inability to pay. This program covers medical, dental, behavioral health, radiology, and lab services, with discounts based on your household size and income.

Purpose of the Sliding Fee Discount Program
Healthy Connections provides discounted healthcare services to eligible families, ensuring that financial barriers don’t prevent access to care. The Sliding Fee Discount policy is a privilege designed to make medical, dental, and other health services more affordable for low and moderate-income households. However, to maintain compliance with federal law, Healthy Connections requires accurate and up-to-date documentation to verify eligibility for the discount. If you disagree with the discount determination, you may file an appeal for review.
How Does It Work?
Discounted services are based on your household’s gross income and the number of people living in your home. These guidelines are set annually by the Healthy Connections Board of Directors to help uninsured and underinsured patients. You can request to have your discount category recalculated if your household income or size changes. If you feel your discount category was incorrectly determined, you can schedule a review with the Chief Financial Officer or Chief Executive Officer.

Who Qualifies?
Types of Discounts Available
- Medical & Behavioral Health: Full-price charges for medical and behavioral health services may be reduced based on your discount category.
- Dental Services: Pre-approved dental services can be discounted based on your sliding fee category.
- Lab & Radiology Services: Lab, x-ray, and ultrasound services are discounted according to your sliding fee category, with specific lab days offering additional discounts.
Eligibility for Sliding Fee Discount
You may qualify for the Sliding Fee Discount if:
- Your household's gross income is less than 200% of the federal poverty level based on current federal guidelines.
- You provide updated proof of income and household size every 12 months.
- Your account is current, and payment arrangements are followed.
Required Documentation
To apply for the Sliding Fee Discount, you need to provide:
- Proof of Income: This could include a tax return, W-2, pay stubs, a signed letter from an employer, unemployment checks, or Social Security benefit statements. Self-employed patients must submit tax returns, while bank statements are not accepted.
- No Income Statement: If you have zero income, you must sign a legally binding “No Income Statement.”
