Help With Medicaid and CHIP
Medicaid provides access to important healthcare services for eligible patients and families, including routine checkups, preventive care, prescription medications, behavioral health care, dental services, and other covered services.
Healthy Connections accepts Medicaid at all clinic locations throughout Western and Central Arkansas. Certified Assisters can help new and current patients apply for Medicaid, complete renewals, and understand next steps if they have received a notice about their coverage.
Medicaid and CHIP applications are accepted year-round. Eligible patients and families do not have to wait for the annual Health Insurance Marketplace Open Enrollment period to apply.

Open Enrollment is the time when many people can choose or update health insurance coverage for the coming year. During the annual Health Insurance Marketplace Open Enrollment period, Healthy Connections Certified Assisters can help patients review coverage options and complete applications.
Many patients may qualify for financial assistance that lowers monthly premiums for Marketplace plans. Our team can help patients understand the application process, review available options, and make sure enrollment steps are completed correctly and on time.
Open Enrollment dates may change each year, so patients are encouraged to call Healthy Connections for current information and appointment availability.
Sliding Fee Discount Program
Healthy Connections offers a Sliding Fee Discount Program for eligible patients. This program is designed to help reduce the cost of care for patients who qualify based on household income and family size.
The Sliding Fee Discount Program may help lower the amount a patient pays for covered services at Healthy Connections. Patients with or without insurance may ask about the program.
Certified Assisters can help patients complete the Sliding Fee Discount Program application and understand what documentation may be needed.
What to Bring to Your Appointment
To help the process go smoothly, patients may be asked to bring documents such as:
- Photo identification
- Medicaid letter or disenrollment notice, if you received one
- Insurance card, if you have one
- Medicaid or Medicare card, if applicable
- Proof of household income
- Information about household size
- Recent letters or notices from Medicaid, Medicare, Access Arkansas, the Health Insurance Marketplace, or an insurance provider
