Psychiatric Mental Health Nurse Practitioner (APRN PMHNP)
In areas where there is limited availability of psychiatrists, the PMHNP extends the availability of cost-effective mental health care. PMHNPs diagnose and treat common acute psychiatric problems, illness, and crisis. The PMHNP assesses the mental health needs of individuals, families, and groups. They formulate healthcare plans, implement treatment, and evaluate the effectiveness of short- and long-term progress. In further detail, PMHNP’s also:

  • Offer outpatient care to address sudden changes in a patient’s mental status or chronic mental conditions
  • Perform various patient assessments
  • Develop holistic, cross-disciplinary treatment plans
  • Prescribe medications in compliance with state scope of practice laws
  • Collaborate with doctors and make appropriate patient referrals
  • Analyze the effectiveness of treatments
  • Education patients, families and others on challenges in mental health
Licensed Clinical Social Worker (LCSW)
LCSWs diagnose and treat mental, emotional and behavioral health issues. They plan treatment, participate in crisis intervention and perform more detailed formal assessments. LCSWs act as advocates and help clients gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse.
Licensed Professional Counselor (LPC)
Licensed Professional Counselors provide mental health and substance abuse care. They are trained to work with individuals, families, and groups in treating mental, behavioral and emotional problems, and disorders.
Licensed Marriage and Family Therapist (LMFT)
LMFTs are mental health professionals trained in psychotherapy and family systems and licensed to diagnose and treat mental and emotional disorders within the context of marriage, couples, and family systems.

coordination with primary care physicians

You do not need to have a Healthy Connections primary care provider in order to take advantage of Evolve Behavioral Health Services and Medication Management. We are a stand-alone program and can work in coordination with your current provider to facilitate treatment.

support groups and group therapy sessions

Support groups bring together people who are going through or have gone through similar experiences. If you’re facing a major illness or stressful life change, you don’t have to go it alone. Talking about your experiences with others may help.
