Healthy Connections is a Patient-Centered Medical Home (PCMH) which is a way ofsaying you, the patient, and your family are the most important persons in the health care system.
A medical home is approach to providing total healthcare. With your medical home you will join a team that includes healthcare professionals, dental care professionals, behavioral health professionals, x-ray services, and most importantly, you and your family.
Healthy Connections has locations throughout west and central Arkansas, including Mena, Hot Springs, and Little Rock.
Most Healthy Connections locations are open from 7:45 a.m. to 6 p.m. Monday through Friday.
After Hours Urgent Needs
If you are experiencing a medical emergency, you should proceed to the nearest emergency room or call 911. If you need non-emergent medical advice after business hours, please dial 1-888-710-8220 and press 5 to be transferred. If the provider is unable to answer, please leave a voicemail with your name and phone number. The provider on call will return your call within one hour. Medications will not be prescribed or refilled after hours.
Our PCMH can help you:
Get to know your team. Your medical home team will know you and your family. They can help you manage your total healthcare. You can see the same team each time you visit and they can help answer your health questions.
Work with other medical experts if needed. For example, if you need to see a specialist, your team can keep in touch with the specialist to make sure you get the care you need.
Track your health information using electronic medical records. This way, your team will have all of your records in one place.
Reach your team when you need them. Your team will have convenient office hours for your appointments and an after hours call service. We also offer you the option of using our Internet Patient Portal to keep in touch with your team.
What can you do for your PCMH?
Be an active team player. Talk with your team about your health questions. Share the successes and the challenges you’ve had with healthcare in the past.
Tell your team about other healthcare professionals who care for you.
Tell your team how you feel about the care you are getting from them.
Take care of your health. Follow the healthcare plan you and your team have worked out. Make sure you understand how to follow the plan.
Set goals you can reach. Once you begin to see results, you and your team can discuss adding new goals.
Talk openly with your team. If you are having trouble sticking with your care plan, tell your team about it—together you can make changes.

Healthy Connections, Inc is a community health center that provides medical, dental, and social services to Arkansas regardless of ability to pay. We offer a sliding fee scale for those who qualify and low cost or no cost services, as well as accept most major insurance carriers. Healthy Connections discounted services are perfect for those that are underinsured or uninsured.

About Healthy Connections
Healthy Connections is here to help improve your physical and mental health and well-being with a commitment to excellence in all that we do.
Our providers see patients related to primary and preventative medical care — from children to senior citizens. We offer same-day appointments and walk-ins are welcome.
We also now offer Behavioral Health services with a licensed therapist in the clinic to provide emotional support, mental health evaluations, therapy, and case management services to people experiencing psychological, emotional, medical, social and/or familial challenges.
Healthy Connections accepts Medicaid, Medicare, and most private health insurance plans. However, no person will be turned away for lack of insurance. Qualified families will be charged according to their total family income on a sliding scale. Enrollment assistance is also available for Medicaid, ARKids 1st and the Affordable Care Act.
